I love the idea of using barley instead of rice. I have used buckwheat before as well. My mom used to make a dish similar to this with cabbage, layered with meat and rice, with some tomato sauce. Same idea (she used to make golabki as well, but yeah, that takes time). I hope your nervous system can settle soon!
